Note: Transport/receiving temperature logs are not being maintained. Unpermitted MFF observed parked in front of facility (Mexico Bakery #06071T3). Per owner, their original MFF (Mexico Bakery #CF55V41) is currently being repaired and this vehicle is being used as a loaner. In addition, the unpermitted MFF is using the facility as a commissary kitchen for food storage, electrical hookup, and warewashing facilities. Owner does not plan on obtaining a permit for the loaner vehicle and stated that original MFF will be fully repaired by Monday, 10/27/25. MFF's food stored in walk in cooler was transported back to commissary kitchen in an ice chest during inspection: 1560 Alum Rock Av. Instructed operator to cease and desist all food operations for MFF #06071T3. Observed operator closing down MFF operations. MFF was removed from the premises. This Department will continue to monitor this address. Further evidence of unpermitted MFF activities may result in further enforcement and/or legal action.

OWNERSHIP CHANGE INFORMATION NEW OWNER: Mexico Bakery Inc. FACILITY NAME: Mexico Bakery Super Mercado #2 Operation start date: 9/8/25 Note: Facility has an open plan check project for kitchen area: SR0874541. No cooking equipment observed in kitchen. Operator will be transporting a variety of prepackaged food items and Mexican sweet pastries from the following facility: MEXICO BAKERY INC 1560 ALUM ROCK AV SAN JOSE, CA 95116 Temperature logs shall be maintained for any potentially hazardous food items. Ensure food items that are packaged at 1560 Alum Rock Av are properly labeled with the following information: 1. The common name of the food. 2. If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food. 3. An accurate declaration of the quantity of contents. 4. The name and place of business of the manufacturer, packer, or distributor. Ensure all juices made at 1560 Alum Rock Av are properly labeled with the information listed above as well as the following "Warning" statement: 5. “WARNING: This product has not been pasteurized and, therefore, may contain harmful bacteria that can cause serious illness in children, the elderly, and persons with weakened immune systems. The applicant has completed the change of ownership application process for an Environmental Health Permit. The permit category for this facility is FP09. An invoice for the permit fee in the amount of $652.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice.
