ROUTINE INSPECTION
1. Observed Vermin: Documented in the following areas: Several rodent droppings observed in the walk in refrigerator, on boxes of beverages, on shelves with food and beverage, and on the floor in several areas throughout the facility. Also dead cockroach on the food shelf. and on the back room shelf. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Anjani Sircar 4. Notification: The person in charge during inspection, Lieu Ly, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.
Observed many rodent droppings on shelves with food items, and on the floor in many places throughout the facility. [CA] Take effective measures to eliminate the rodents from inside the facility. Close any holes in the walls and ceiling. Remove food/beverage from the floor. Facility must close and follow the checklist before calling for a reinspection to request opening of facility.
Observed boxes of beverages with rodent droppings on them and droppings in the refrigerator. [CA] Discard any food/beverage contaminated by rodents. Take effective measures to eliminated the rodents from inside the facility. Close any holes in the walls. Remove food/beverage from the floor and clean and sanitize.
FOLLOW-UP INSPECTION
Reinspection due to observation of droppings on the floor in the storage room. The storage room was cleaned up. The back small is full and disorganized. This room must be cleaned and organized. Also, continue to clean the sales shelves of spills and debris.
ROUTINE INSPECTION
Observed the walk in refrigerator to have a soiled floor and shelves. [CA] Thoroughly clean the floors and shelving inside the unit.
Observed rodent droppings on the floor and on a storage shelf. [CA] Thoroughly clean up the droppings and protect food from contamination. A reinspection will be done. According to the manager there is not a current rodent problem. Be sure to cover any holes in the walls.
The back door has a gap under it. [CA] Close the gap under the door to help eliminate insect and rodent entrance.
