    Burritos were in the warmer at 110 degrees F. According to the employee they are supposed to be put in the oven first not the warmer. [CA] Reheat the burritos to 165 degrees F before placing on the hot holding unit. [COS] corrected- put into oven

[CA] The self serve containers must be under the sneeze gaurd or placed into small containers with the lid similar to the containers of salsa. Protection from overhead contamination must be used. [COS] The employee placed them under the sneeze gaurd and stated that they may begin putting it into small containers with lids.

    The open bulk food bag of sugar is on the storage shelf. [CA] All open bulk food bags must be stored in bulk food bins with lids. [COS] The employee placed it into a metal container with the lids covering temporarily.

    The ice machine has black spots forming inside on the upper panel due to mildew-like growth. [CA] Thoroughly clean and sanitize the inside of the machine.

    The drain pipe behind the ice machine is in the floor sink. [CA] Raise the drain pipes to provide an air gap between the top of the floor sink and drain pipe.

    Observed the automatic dispenser to give 0ppm sanitizer. The employees attempted several times to make the sanitizer in the bucket which also gave a 0ppm quat reading. [CA] The quat sanitizer must be at 200ppm. If using chlorine the chlorine test strips must read 100ppm chlorine.
