In the refrigerated food section, observed garbage, food debris, and dust accumulation below refrigeration. On food display shelves, observed food debris on shelving. [CA] Ensure facility floors and shelving are frequently cleaned and maintained to prevent harborage of vermin. In the back storage areas, observed an open vent from the roof entering into the area. Vent from the ceiling was not sealed. [CA] Ensure any entrances into the facility are properly finished and sealed to prevent an entrance for vermin.

By the side door, towards the refrigerated food section, observed rodent droppings behind a 2 door reach in refrigerator. [CA] Ensure facility is kept free of vermin to prevent possible contamination of food.

In the employee restroom, observed paper towels stored outside of the paper towel dispenser. [CA] Ensure paper towels are provided from a paper towel dispenser to allow for proper handwashing.

Observed infant formula stored on retail shelving after the "use by" date. [CA] Baby food or infant formula shall not be sold or offered for sale after the "use by" date.
