1. Observed broken wall and missing cove base at the back door by the cookline. [Corrective Action] Maintain the wall in good condition. 2. Oil, grease and old food debris observed on the wall and pipes behind the cooking equipments at the cookline. [Corrective Action] Thoroughly clean and maintain clean.

Pressurized cylinder at the service area observed not secured to a rigid structure. [Corrective Action] Secure the pressurized cylinder to a rigid structure. [Corrected on Site] PIC secured the pressurized cylinder.

1. Observed cracks at the pre-wash sink at the dishwasher area causing water to leak to the bottom. [Corrective Action] Repair or replace the pre-wash sink. 2. Observed interior metal lining in the 3 door prep cooler separating from the insulation causing food and debris to become stuck in the crevice. [Corrective Action] Repair the metal lining inside the 3 door prep cooler. The 3 door prep cooler may need to be replaced.
