In the back storage area by ice machine, observed gaps in the ceiling due to misaligned panels. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage.

Observed active leak and accumulation of standing water underneath the ice machine. [CA] Repair leak in ice machine. Equipment shall be maintained in good repair.

Observed drainpipe from the 3-compartment sink extends directly into the floor sink. [CA] Provide air gap that is at least one inch or twice the diameter of the inlet. [COS] PIC elevated the pipe during inspection.
