OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: HANA BOTTLE SHOP NEW OWNER: HANA BOTTLE LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $777.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 09/01/2025 - 08/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. * Structural Review inspection conducted on 08/21/2025. * In the self-service area, facility has a Bunn ("Siberian Chill") frozen slushie machine. Confirmed with PIC that the facility has a contract for Pepsi to routinely fill, maintain, and clean the slushie machines. Pepsi provides monthly services to facility. The facility will not conduct any food preparation on-site.

- K30: Food storage: food storage containers identified
Observed beverages boxes stored directly on the floor in the walk-in cooler. [CA] Food shall be stored six inches above the floor to prevent contamination.
- K35: Equipment, utensils: Approved, in good repair, adequate capacity
Observed the active use of three coffee machines and grinders and domestic microwave in the self-service area. Under the current permit, facility is not approved for open food preparation and lacks a warewashing sink. [CA] PIC agreed to immediately discontinue the use of coffee machines and microwave. The coffee machines and microwave will be removed from the premises. If facility wishes to continue the service of coffee, prior approval and submittal of plans will be required by this Department to install a warewashing sink.

Observed lack of thermometer in the refrigeration unit in the back (holding dairy and Potentially Hazardous Foods). [CA] The thermometer in the reach-in refrigerator shall be placed where it is easily visible while checking refrigerator temperature.

Observed gap between the bottom of the back door and the ground, leading to the outside of facility. Observed missing or broken ceiling panels in the storage area. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage.
- K36: Equipment, utensils, linens: Proper storage and use
Observed pressurized cylinder tanks are not securely stored. [CA] All pressurized cylinders shall be securely fastened to a rigid structure.

Measured the maximum water temperature at the janitorial sink at 114F. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks.
