Observed cleaning chemicals stored on a shelf above food wraps/liners. [CA] Store all cleaning supplies separate and below all other items.[COS] Employee corrected the issue.

Observed a live roach on the glue board. [CA] Continue to take effective measures to eliminate the cockroaches from inside the kitchen. Discard the glue board with the roach on it and replace with new board. Protect all food from possible contamination.

The walk in cooler has not been used in several years according to the employees. It is dusty, has spider webs, stains on shelves and door frames. [CA] thoroughly clean the inside of the walk in, including but not limited to the floors, shelves and door frame. Kitchen items are stored in the walk in.

There ia a gap under the screen door. The door is usually left open per the employee. [CA] Install weather stripping to the bottom of the screen door to help keep insects out of the kitchen.
