OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: 7-Eleven 14253D NEW OWNER: Pinnacle Retail Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP09. An invoice for the permit fee in the amount of $652.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 06/26/2025 *Permit condition: TurboChef oven approved for heating precooked food items only. If any sanitation, ventilation, or safety problem, the exhaust hood exemption may be revoked and an approved mechanical exhaust system must be installed, or the unit removed. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.

An approximate 2ft by 3ft section of the flooring inside the restroom observed replaced with wood. [CA] The floor surfaces shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable. Make plans to replace/remodel facility with approved flooring. Observed walls in the back storage behind the three-compartment sink composed of wood. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface. Observed square hole in the ceiling by the roof access ladder. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Heavy accumulation of debris observed on floors in the back storage area, especially underneath sinks and shelves. [CA] Walls and/or floors shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

Pico de gallo maintained at the top insert of the food preparation refrigerator for condiments measured at 47F. Numerous packages of taquitos and hot dogs maintained inside the same refrigerator measured at 45F. Ambient temperature of the refrigerator measured at 45F. Thermostat on the outside displayed a temperature of 38F. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] PHF items inside the refrigerator were relocated into the walk-in refrigerator to immediately cool.

Hand wash station at the cashier lacked hand soap dispenser. Hand soap was provided in a squeeze bottle. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times.

Cases of food and beverages maintained directly on the floor. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

Ambient temperature of the food preparation refrigerator for condiments measured at 45F. [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below.

Two dead cockroaches observed in-between the shelving where the beverage dispensers are located on the sales floor. No other activity was observed. [CA] Clean and sanitize area of dead cockroaches or old droppings.
