Observed potentially hazardous food that measure in the temperature danger zone located in the food prep cold hold unit (chicken, beef, pork 56F) the reach-in cooler (beef 52F), and on a food prep surface (bean sprout 78F). [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Repair, adjust or replace units if necessary. PIC will have staff use time control and discard the PHFs at 6pm. Facility must send an invoice or proof that unit has been fixed by 7/30/2026 to the inspector. [SA] PIC time marked the food as a suitable/temporary solution.

Lack of air gap between food prep sink's discharge pipe and the floor sink. [CA] All equipment that discharges liquid waste shall be drained by means of indirect waste pipes, and all wastes drained by them shall discharge through a 1" airgap (minimum) into a floor sink or other approved type of receptor.
