    Observed dead cockroaches in sticky traps in the bar area. [CA] Eliminate vermin from facility; work with licensed/professional pest control. Clean and sanitize area; set monitoring traps.

    Observed potentially hazardous foods located in the left food prep cold hold unit that measured in the temperature danger zone (see measured observations). Ambient temperature reading was 46F. Staff stated that the PHFs were put into the unit 30 minutes prior. [CA] Ensure PHFs are properly cold held at 41F or below or hot held at 135F or above. Repair, adjust or replace unit if necessary. [COS] PIC called repairman to fix and had staff relocate PHFs to the walk-in cooler.

    Chlorine at bar warewash machine measured at 0ppm. [CA] Ensure chlorine in mechanical warewash machines measure at 50ppm.

Follow-up inspection for violation 23; owner said they caught 4 rats, sealed entry points, and set 42 traps and will continue monitoring. Facility has been cleaned and sanitized with a diluted bleach solution. Facility will continue working closely with pest control. Observed a few droppings that were missed during cleaning; facility subject to a charged follow-up inspection.
