    The floors and walls are deteriorated in several areas throughout the facility. [CA] Repair the walls and repair/replace deteriorated flooring.

    Observed several beverages on the floor. [CA] Remove all beverages from the floor. No food or beverage may be stored on the floor. Protect food from possible contamination.

    The refrigerator with the milk in it does not have a properly working thermometer in it and the digital one is not giving the temperature. [CA] Provide a thermometer inside the unit.

    Observed unused refrigerator unit and freezer chest and icee machine. [CA] Remove the unused equipment. The Endura refrigerator has debris on the bottom inside the unit. There is a cardboard also on the bottom inside. [CA] Remove the cardboard and clean the inside of the unit.

    Observed rodent droppings on the floor in the corner near the boots display. [CA] Thoroughly clean and sanitize the areas. Take effective measures to eliminate rodents from inside the facility. According to the employee he believes they must be old since no activity observed and they do have a pest control company.

Follow-up inspection for major violation #23. Observed a few old rodent droppings in both unused walk-in coolers in the back area, underneath the shelving on the main sales floor, and a few by the doorway of the wooden unused storage room. Facility will have an additional follow-up inspection that is subject to be charged to ensure cleaning is complete ($298.00/HR). OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: MD MARKET INC dba MADRONE MARKET NEW OWNER: MD MARKET INC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $777.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 10/01/2024 - 09/30/2025. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
