FOLLOW-UP INSPECTION

Follow up inspection for facility closure due to cockroach infestation. Observations: K23: One dying cockroach was found near the service area. Staff immediately eliminated the cockroach. One dead cockroach was observed on the floor under the soda machine on the employee side. Vermin proofing measures were observed such as sealing wall gaps in the back of house area. Continue to vermin proof the facility. Facility was serviced by Terminix at 7:21 am 01/15/2025. Per service record they performed a crack and crevice treatment. Note: A contractor arrived on site to repair the gooseneck faucet and address other structural concerns (FRP, tiles, vermin proofing). Facility is okay to operate. A follow up inspection may be conducted to determine compliance. The first follow up inspection is free of charge. Any and all subsequent follow up inspections shall be charged at the approved hourly rate of $298.00/hr, minimum of one hour, during normal business hours: Monday through Friday 7:30am to 4:30pm or $645/hour minimum two hour during non business hours, and upon inspector availability.

In the kitchen area, in an outlet near toaster ovens in between the flat grill and fryers, a live nymph and hatched ootheca was observed. 5. In the back of house area, near the wooden pallets by the soda syrups, a hatched ootheca was observed. 6. In the dustpan, numerous dead cockroaches were observed. Per PIC, they swept them up from cleaning outside. 7. At the front service counter, on the pillar near the paper napkins, an adult cockroach was observed. Notes: Facility has monthly pest control from The Pied Piper, [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
