    Repeat Violation: In the back of house area, near dry storage ware house racks, old rodent droppings were observed. [CA] Ensure to maintain the facility free of animals, pests, and vermin. Clean the area of old rodent droppings. [COS] Staff cleaned the old droppings at time of the inspection.

    Both restrooms lacked toilet tissue in the dispenser. Toilet tissue was available on the toilets. [CA] Toilet tissue shall be provided in a permanently installed dispenser at each toilet.

    Several of the sales floor aisles had food debris and products under the shelving. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.

    1. In the produce area, at a vegetable display area, small flies were observed. 2. In the back of house area, near dry storage ware house racks, old rodent droppings were observed. 3. In the salesfloor area, under the metal shelves of bread, on a glue trap an old small dead rodent was observed. Note: reviewed pest control reports. Latest report from 10/06/25 indicated no activity. No other evidence of rodents were observed in the way of gnawing, rub marks, nesting, urine, fresh dropping, or live bodies. [CA] Ensure to maintain the facility free of animals, pests, and vermin. Clean area of dead droppings and body of rodent. A follow up inspection will take place to ensure compliance.

    The customer restroom lacks a soap dispenser but had a bottle of soap available for use. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times.

    1. In the produce area, at a display area of fruit, fruit flies were observed. 2. In the outdoor refuse area there were numerous flies found. [CA] Ensure to maintain the facility free of animals, pests, and vermin. Provide pest control as necessary.
