The rear storage room has gaps along with base of the floor. [CA] Ensure to maintain the floors of the facility in good repair. Seal the gaps to preent vermin entrance and harborage.
Two dead oriental cockroaches were observed on a sticky trap under the two compartment sink. [CA] Clean and sanitize area of dead cockroaches.

Notes: - Per PIC, the hose that discharges to the hand sink in the restroom is for the A/C unit. Move the liquid discharge to an approved sewage draining system (floor drain, floor sink, mop sink). - Facility lacks a mop sink, any remodeling or change in operation will require plan check submittal and installation of a curbed cleaning facility or janitorial sink.
Facility has a liquid pesticide stored under the two compartment sink. [CA] Only those insecticides, rodenticides, and other pesticides that are necessary and specifically approved for use in a food facility may be used.
The rear storage room with electrical equipment has several holes in the walls and ceilings. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.
