Any and all subsequent follow up inspections shall be charged at the approved hourly rate of $298.00/hr, minimum of one hour, during normal business hours: Monday through Friday 7:30am to 4:30pm or $645/hour minimum two hour during non business hours, and upon inspector availability. - Per PIC, Siriphorn, she plans on replacing the combo 8 burner range/two ovens/and flat grill unit with another 8 burner range and a separate wok grill. Ensure to submit plans to our plan check unit.     1. The two door prep cooler is not maintaining PHFs at 41°F or below. Note: Per PIC, they have purchased a replacement unit [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below. 2. The two microwaves in the kitchen and the two freezers are not ANSI accredited. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation).

    Chlorine solution at the three compartment sink measured at 0 PPM while dishes were in the solution. [CA] Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM). [COS] Solution was remade and measured at 100 PPM.

    1. Repeat Violation: Cove base missing near handwash sink in the kitchen area. Damaged walls observed in kitchen area. [CA] Repair cove base and wall structural damage to prevent pest harborage. 2. The stainless steel paneling near the cookline area is beginning to lift. [CA] Ensure to maintain the floors, walls, and celings of the facility clean and in good repair.

  • K36: Equipment, utensils, linens: Proper storage and use

    1. Scoopers for the rice were found in standing water measuring at 75°F. [CA] Utensils shall be stored in the following manner: 1) in the food with their handles above the top of the food and the container, 2) in running water, 3) in a container of water at 135°F or above, 4) at room temperature if washed within 4 hours of initial use. 2. Facility is using bowls as a scooper for their cooked rice. [CA] Use approved utensils such as scoops or spoons with handles to ensure hands do not come in direct contact with food.

  • K21: Hot and cold water available

    Water at the men's and women's restroom handwash sinks were measured at 70°F and 72°F degrees respectively. [CA] Handwashing facilities shall be equipped to provide minimum 100°F water under pressure for a minimum of 15 seconds.

    1. Garlic in oil mixture was found stored on the wire rack next to the cookline equipment and measured at 70°F. Per staff, it has been there since at least yesterday. 2. Tofu, Fried Tofu, and Bean Sprouts in the prep cooler measured about 60°F . Per chef, they placed it there about an hour prior. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] 1. Garlic in oil mixture was discarded. See VC&D report. [COS] 2. PHFs were moved to working cooler.

    Cooked chicken was found commingled with raw chicken in the single door prep cooler. Cooked Chicken measured at 61°F and raw chicken measured at 41°F Note: Per staff, a customer changed their order and it was placed back into the raw chicken. [CA] No employee shall commit any act that may cause the contamination or adulteration of food, food-contact surfaces, or utensils. [COS] Chicken was discarded. See VC&D report.

Last pest control service report conducted on 11/18/24 was reviewed. No cockroach activity noted on report. Per operator, pest control services are provided twice a month.

    Multiple knives stored in between preparation refrigerators. [CA] All cleaned utensils shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least six inches above the floor.

    Employee shoes observed under kitchen shelf. [CA] Personal belongings shall be stored in a designated area that will prevent the potential contamination of food and food contact surfaces.

    Multiple drain flies observed in kitchen area. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.

  • K07: Proper hot and cold holding temperatures

    In the two door preparation refrigerator, sliced tomatoes stored in insert measured at 45F. Per operator, tomatoes were placed in inserts less than 2 hours prior. [CA] PHFs shall be held cold at 41°F or below. [SA] Sliced tomatoes were relocated to upright cooler.

  • K39: Thermometers provided, accurate

    Facility is lacking a metal probe thermometer. [CA] An accurate easily readable metal probe thermometer (accurate to +/- 2°F) that is designed to measure the temperature of thin masses shall be provided and readily available to accurately measure the temperatures of potentially hazardous foods.

  • K06: Adequate handwash facilities supplied, accessible

    In front service area, tray stored in handwash sink basin. [CA] Handwash stations shall be clean, unobstructed, and accessible at all times.

  • K41: Plumbing approved, installed, in good repair; proper backflow devices

    In the 3 compartment sink, leak observed on drain pipe under the third compartment. [CA] Repair leak and maintain in good repair.
