FOLLOW-UP INSPECTION

Follow-up inspection for possible vermin activity noted on previous routine inspection. Reviewed latest pest control service report dated 2/6/26 that notes three cockroach activity was observed in the kitchen and has been treated. At time of inspection, one dead cockroach was observed under the dishwasher and operator notes that live activity has been seen under the prep sinks. No live activity was observed by inspector. Facility shall obtain an additional pest control service within the week and a subsequent and charged follow-up inspection will be conducted. Any signs or evidence of an infestation during the inspection may result in enforcement action. ***Subsequent follow up inspection after first follow up shall be billed $298/hr during business hours and $645 for a minimum of 2 hours, during nonbusiness hours upon availability.***

Did not observed live of dead cockroaches elsewhere in the facility or any other signs of an active infestation. Reviewed facility's most recent pest control service report dated 1/8/26 where activity was noted and was treated. Operator will provide district specialist a copy of next pest control service report for review and a follow-up inspection may be conducted.
