Missing placard and unaware to what happened to it. [CA] The placard shall be posted and clearly visible to the general public and patrons entering the food establishment. NOTE: Facility has a break in and therefore has to replace door and lost placard.

Observed plastic wrap and dust debris under shelving in the back area and inside the walk in cooler. [CA] Clean and sanitizer flooring all over the facility and keep it maintain.

Facility lack soap dispenser and has empty paper towel dispenser. Facility using pump soap and paper towel roll. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. Provide handwashing cleanser in dispenser at handwash stations at all times.

Half of the front door was kept open. [CA] Keep door closed at all times to prevent the entrance and harborage of vermin.

Cardboard in use to line floor under drinks inside the walk in cooler. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

Damaged floor near the ice machine and in the store area. Damaged wall outside the restroom. [CA] Repair the floor & wall with a durable, smooth, nonabsorbent, and easily cleanable surface.
