    Food Handler's Card lacking for a couple employees. [Corrective Action] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

    Observed mold like substance inside of the ice machine near the ice chute. [Corrective Action] Thoroughly clean and sanitize inside of the ice machine. Recommend creating a cleaning schedule.

    1. Multiple unused equipments observed at the rear of the facility. [Corrective Action] Remove and unused equipments from the premise. 2. Observed accumulation of flattened cardboard boxes stored next to the backside of the building. Per manager, the cardboards are removed weekly. [Corrective Action] Store flattened cardboards away from the building. Recommend operator to obtain a recycling bin.

Verified with owner that facility on 1006 E Santa Clara Street, San Jose is no longer operating. Per owner, business is sold to a columbia cuisine. Owner requested to inactivate permit on 1006 E Santa Clara Street, San Jose

    Dishmachine sanitizer concentration measured at 25ppm. [CA] Maintain chlorine in mechanical warewash machine at 50 PPM. PIC contacted dishmachine company to maintain and adjust dishmachine.

  • K41: Plumbing approved, installed, in good repair; proper backflow devices

    Minor leakage from dishmachine. [CA] Repair leakage.

  • K34: Warewash facilities: installed/maintained; test strips

    Missing test strip to test dishmachine sanitizer concentration. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

    Faked dog sausage (number 2) on menu. Per PIC, sausage is actually made from pork. No dog meat inside. Sausage is made in facility. [CA] Food offered for human consumption shall be honestly presented in a way that does not mislead or misinform the consumer.

    Plastic bags were used to store meat in freezer. [CA] Use food grade containers or material to store food items.

    Only compost/ organic dumpster and grease collection bin observed on site. Observed piles of cardboard stored outside of facility. Per PIC, trash was discarded to compost/organic dumpster. Cardboard was stored and take to recycle company for money. [CA] Provide trash pick up service. Observed facility uses plastic and items that are non compost and not recyclable.
