Repeated violation: The ice scoop for the ice machine was stored on unclean surface. [CA] All reusable utensils including the ice scoop must be stored on clean surface/container in between use.
- K43: Toilet facilities: properly constructed, supplied, cleaned
The exhaust vent cover for the restroom has been removed. [CA] Replace the missing screen cover immediately. There was no toiled paper in the restroom. [CA] Refill the toilet paper immediately.

The ceiling back in the storage area is falling off. [CA] Replace the damaged/broken or missing ceiling tiles immediately.

The drainpipe from the ice machine was immersed inside the floor sink. [CA] To prevent possible contamination from backup of sewage, please provide at least one inch air gap between the floor sink and the drainpipes.

Heavy mold-like buildup was observed inside the ice machine’s internal part. [CA] Dispose the ice and conduct a thorough cleaning of the ice machine immediately. The PIC dispose the ice and cleaned the ice machine (COS).

There is no wet mop hanger by the janitorial sinks. [CA] Install a wet mop hanger by the janitorial sink. All wet mops must be hanged on the wet mop hanger.
