Hand wash station in the restroom lacked hand soap and paper towels. Facility does not have any other dedicated hand wash station. Hand soap and paper towel dispenser provided at the three-compartment sink. Hand soap dispenser also observed empty with no other alternative. [CA] Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. Ensure hand soap and paper towels are easily accessible and readily available at all times. [COS] Hand soap and towels provided. ** Repeat violation.

Hand wash station inside the restroom lacked warm water. [CA] Handwashing facilities shall be equipped to provide minimum 100°F water under pressure for a minimum of 15 seconds.

Hands are washed at the three-compartment sink. Hand soap dispenser lacked hand soap. A large squeeze hand soap bottle was provided and maintained on the left side drain board of the three-compartment sink.
