    Lack of green placard on the window. [CA] The placard shall be posted and clearly visible to the general public and patrons entering the food establishment.

    The facility has installed three compartment sink, hand wash sink, mop sink, new kitchen space with hood, new floors and multiple pieces of new equipment. Plans were not submitted by the facility. New kitchen or new equipment is not used at this time. Currently facility is only selling coffee, prepackaged foods and drinks. The permit category will need to evaluated and upgraded. [CA] A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: WOLFE LIQUOR NEW OWNER: SUNNYVALE FAST FILL MARKET #2 The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $777.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 01/01/2025 - 12/31/2025 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
