    Two dead cockroaches observed to the side of the ice machine. [CA] Food facility shall be kept free of vermin and vermin debris. Clean and sanitize the affected area. Facility has monthly pest control service but latest service report was not available at time of inspection. Reviewed service report for May and June of this year. Reports notes targeted pest but no additional details were provided. Did not observe cockroaches (dead or live) or other signs of an infestation elsewhere in the facility.

FOLLOW-UP INSPECTION

Follow-up inspection to routine inspection conducted on 4/28/25 which resulted in a closure due to vermin. A professional pest control service was conducted on 4/28/2025 at 12:40 pm. Per pic a contact time of 3 to 4 hours was allowed prior to cleaning. No live cockroaches observed at the time of inspection. The potential entry and vermin harborage points have been repaired: 1. Rear door to the facility adjusted and door sweeps added. 2. All drain clean-out covers sealed/ replaced. Floor drain screens replaced. Continue to work on minor violations noted during the routine inspection. Facility is reopened and may resume operation.

ROUTINE INSPECTION

Facility is closed due to live cockroach activity within the kitchen. Prior to reopening facility shall conduct the following: 1. Cockroach activity is eliminated. 2. All contaminated surfaces are cleaned and sanitized. 3. All contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. 4. A professional pest control service is received. The pest control service report shall specify what treatment chemicals have been applied, application time and when areas will be safe to re-enter. Written approval must be received from this department prior to reopening. To request a follow-up inspection to verify correction, call Travis Ketchu at (408) 918-3469, Monday-Thursday 7:30 AM-5:00 PM, call (408) 918-3400 on Fridays (office is closed on County holidays). The first follow-up inspection is free of charge. Subsequent follow up inspections after first follow up shall be billed $298/hr during business hours and $645/ hr during non business hours upon availability. Failure to comply may result in enforcement action.
