  • K45: Floor, walls, ceilings: built,maintained, clean

    Accumulation of food debris in floor sink and grime on floors at the front. [CA] Clean the floors and floor sinks and maintain in good condition.

  • K23: No rodents, insects, birds, or animals

    Observed 2 dead cockroaches on sticky trap next to under counter refrigerator at front sushi prep. When attempted to inspect another sticky trap in the front area, PIC removed the trap and discarded it and said traps are old. Not able to verify if there was any activity on the other trap. No other activity observed in the facility. Per PIC, pest control services from Banner Pest Control are obtained twice a month. [CA] Clean and sanitize area of dead cockroaches. Discussed with PIC: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately.

  • K07: Proper hot and cold holding temperaturesCritical

    Measured rice for sushi at 60F in a warmer container at front sushi prep area. Per PIC, rice was prepared at 9:45 AM. [CA] PHFs shall be held at 41°F or below or at 135°F or above. OR [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] PIC provided a time sticker for sushi rice to use or discard any leftovers within 4 hours from when it was taken out of temperature control.

  • K40: Wiping cloths: properly used, stored

    Soiled wiping cloth used on the prep counters without sanitizer. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium).

  • K01: Demonstration of knowledge; food safety certification

    Lack of food handler card for employee in the facility. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.
