ROUTINE INSPECTION

Routine inspection. See CO0159255. The was facility was closed due to evidence of an active cockroach infestation. Facility will remain closed at this time and will not reopen until a follow-up inspection is conducted. Facility is to cease and desist all food sales and operations, including food preparation. Do not remove, cover-up, or relocate red placard. Failure to comply may result in enforcement actions. Provided Checklist for Reopening After Closure Due to Vermin Infestation. Clean and sanitize all food and nonfood contact surfaces. Remove all evidence of cockroaches from facility and ensure there is no evidence of vermin found at follow up inspection. ***If ANY cockroaches, dead or alive, are observed during the follow-up inspection(s), the facility will not be re-opened. *** Requirements before scheduling a reinspection (a minimum 24 hours must elapse before our office can schedule a reinspection): 1. Complete and sign the Reopening Checklist. 2. Obtain a copy of the latest pest control report from a licensed provider. 3. Email the TWO ITEMS (Checklist and professional pest control report) to the main office for evaluation. 4. If the checklist and pest control reports meet reopening requirements, then a reinspection will be scheduled. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. Due to technical difficulties, report was emailed to PIC during inspection.

Observed cockroaches in the following locations: - 1 live cockroach (nymph) on the floor in the front service area, in front of a large cabinet - 1 live cockroach (nymph) on the floor in the rear kitchen area, near the center prep counter - 2 dead cockroaches on the floor under the 3-compartment sink - 1 dead cockroach on the floor between cupboards in the front service area - 1 dead cockroach on the floor in the front service area, near the beverage refrigerator 2. Notification: The person in charge during inspection, Veasana, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1.     Observed accumulation of food residue/debris on floors throughout rear kitchen area. [CA] Keep floors clean.

  • K35: Equipment, utensils: Approved, in good repair, adequate capacity

    1) Observed large ADVANTCO cooker being used to cook dish in rear kitchen area. PIC stated the food is for their breakfast menu and they just started making it today. [CA] All equpment must undergo plan review with Plan Check before installation and use. 2) Observed the prep unit and ice machine were not in use; observed nonfood supplies stored inside. [CA] Equipment must be used for their intended purpose, or removed from facility to prevent vermin harborage.

    Observed the restroom door lacked a self-closing device. Observed toilet paper roll stored on top of toilet roll holder (holder was empty). [CA] Install a self-closing device on the restroom door. Store toilet paper properly in its dispenser/holder.

    Observed a bakery pan inside the hand sink in the kitchen. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Pan was removed from sink.

    Observed food residue on floors and walls throughout facility. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface.

    Observed dry food encrusted in exterior surfaces of balking equipment. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

    Observed numerous small flour insects (beetles) inside the large metal flour bin; no contamination of flour observed (flour is inside the paper bag. [CA] Remove all insects from facility using approved pest control methods. [COS] PIC, removed insects and cleaned and sanitized bin during inspection. Inspect food regularly to ensure no contamination of food occurs.

    Observed multiple large open flour/sugar bags in kitchen. [CA] Food facility’s or manufacturer’s dating information on foods may not be concealed or altered.

    Observed non operational small ice machine and prep table. [CA] Repair equipment or remove from facility if no longer using to prevent vermin harborage.
