[CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Employee refill the paper towel dispenser. Box of frozen shrimps were inside the hand washing sink. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Box of shrimp was taken out.

    Multiple broken floor tiles noted around the kitchen area and hole within the wall noted at the cove base by hand wash station. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Also replace broken floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable.

    Rice within metal tray is covered completely at the time of inspection. [CA] When cooling PHF's, food may be left uncovered or loosely covered to allow rapid cooling.
