Multiple food items were measured above 41F in the cold holding unit but the ambient temperature was below 41F. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] After discussing with PIC, learned food in the process of preparation was put back in for inspection.
- K36: Equipment, utensils, linens: Proper storage and use
Small bowl in use to scoop rice. [CA] Use approved utensils such as scoops or spoons with handles to ensure hands do not come in direct contact with rice. Handle of ice scoop was touching ice. [CA] Handle to ice scoop shall be stored so that it is not in direct contact with ice. Cardboard in use to line shelving. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.
- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
Back door and screen to food prep area was open. [CA] Keep back door closed at all times to prevent the entrance and harborage of vermin.

Some employees can't provide food handlers cards. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire. [SA] Keep copies of all food handlers card at the facility.
- K34: Warewash facilities: installed/maintained; test strips
Lack sanitation test strips. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing. [SA] Obtain test strips for both chlorine and quaternary ammonia.

Container of noddles and boxes of vegetables were on floor. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.
