    Observed one live cockroach in trap in the dry storage room. [CA] Eliminate vermin from facility. Work with professional/licensed pest control and provide report before follow up on 10/17. Recommend setting additional monitoring traps. [COS] PIC killed live cockroach and discarded.

PIC stated that the tray of beef tacos was removed from the warmer for the first round of lunch service. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Repair, adjust or replace unit if necessary. Facility may consider using time control (TPHC- Time as a Public Health Control) provided they fill out the Department's TPHC form, time mark the food for 4 hours and discard at the end of the 4 hours (or earlier). [SA] PIC time-marked the food and will discard.

    Observed one live adult cockroach in trap, and 2 dead cockroaches in the same trap. Also observed dead cockroaches in the light fixtures. [CA] Eliminate pests from facility and clean/sanitize all surfaces. Recommend working with professional pest control and set up at least 10 additional monitoring traps. School shall not serve open food until follow-up inspection from the Health Department and approval to resume serving open food; facility can serve prepackaged only. [COS] PIC killed live cockroach and discarded trap.
