NEW FACILITY NAME: Long Rich Plus NEW OWNER: Long Rich Plus Inc The applicant has completed the application for an Environmental Health Permit The permit category for this facility is FP11. An invoice for the permit fee in the amount of $1580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 10/1/25 - 9/31/26. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Permit condition: Facility is conditionally approved to operate without a floor sink for the food prep and dish washer. Must provide standard of operations (SOPs) for secondary containment for the food prep sink. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 9/17/25.* Plan check consultation is available Monday - Friday from 7:30am - 10:00am in person or by phone at 408/918-3400. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date. A routine inspection will be conducted within 30 working days. Tentative opening date is 11/1/25. - Operator provided SOP.

1. Found flooring in prep/warewash area is exposed concrete and not approved. Observed no coved tile throughout food facility. [CA] Floors must be smooth, clean, durable, and of approved materials. Facility is hereby given 3 months to provide a sample of what kind of flooring they want and then completely install flooring. 2. Found walls and ceiling panel with discoloration and peeling. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface. Walls and/or floors in food preparation area shall be kept clean.
