OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Grewal Food & Liquor NEW OWNER: BKK Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $971.25 ($777.00+25% late ) will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/01/2025 - 11/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. Facility has raised mop sink in back with cut drain line. Raised mop sink drains directly onto the ground outside and into public storm drain. [CA] Mop water shall be disposed of in the janitorial sink or toilet facility that if plumbed into sanitary sewer system. [COS] Per operator, white in ground mop sink to the right of raised mop sink is used. In ground mop sink observed to be draining and is not draining into storm drain. Per operator, sink is shared with facility next door. Facility shall have access to shared mop sink with laundromat next door while in operation; if access to mop sink is lost facility must submit plans to this agency for installation of new mop sink.
