Follow-up inspection to routine inspection conducted on 5/06/2025 and follow-up inspection on 5/09/2025. Received a payment of $645.00 for an after hours/ weekend inspection. Facility received professional pest control services and performed deep cleaning. No evidence of vermin observed at the time of inspection. Continue to work on minor violations noted on the routine inspection and eliminating vermin harborage points. Facility is reopened and may resume operation.

FOLLOW-UP INSPECTION

Follow up inspection regarding facility closure on 05/06/2025 for cockroach activity. Facility ordered to remain closed due to live cockroach activity. Discussed re-opening procedures, vermin proofing and cleaning procedures. Discussed with PIC that today is their first follow-up inspection. Subsequent follow-up inspection will be charged a fee of $298 per hour. After hours follow up inspection shall be charged at $645/hour, minimum two hours, during non-business hours, and upon inspector availability. Red placard posted at front window. Red Placard must be displayed until facility has been re-inspected, and cannot be removed, relocated, defaced, marred, camouflaged, or hidden so as to prevent the public from observing it. Specialist observations: -1 live cockroach activity -hand sink at bar has been repaired and no leak was observed -prep line cooler's top well ambient temperature measured at 50F. PIC stated unit will not be used until it is able to properly cold hold Potentially Hazardous Food at 41F or below.

Pest control report from Western Exterminator Company observed which occurred on 05/08/2025. Report stated "cockroach activity found I'm sushi bar, kitchen and dishwashing area." FACILITY ORDERED CLOSED due to evidence of live cockroaches. Facility shall contact Specialist for a follow up inspection to reopen after all evidence of vermin and vermin activity have been removed and all impacted areas have been properly cleaned and sanitized. Facility shall not conduct food preparation and food sale until facility has obtained approval from Specialist to re-open. Operator shall make available proof of pest control report prior to contacting Specialist for a follow up inspection.
