    Observed potentially hazardous food that had been time marked for 4 hours but were not thrown away at the 4 hour mark (tomatoes 61F and cheese 66F). [CA] Ensure PHFs that are subject to time control are discarded at the end of the 4 hour time mark. [COS] PIC voluntarily discarded the tomatoes and cheese.

    Observed build up of food debris and grime on floors and walls. Observed one wall to not have cove base tiles. [CA] Deep clean and sanitize floors and walls and maintain on a regular basis. Install approved cove base tiles.
