Reinspection for inspection conducted on 06/26/2025. Manager, Lora, requested follow up inspection by email and phone. 1. Worksheet has been received from person in charge. 2. A report from a licensed pest control company has been received and reviewed (service dated on 6/30/2025). 3. All sanitation and food safety items on worksheet have been verified as completed. 4. All prevention measures on worksheet has been verified as completed. No sign of active cockroach and rodent activity at the time of inspection. No monitoring traps were set up at the time of inspection. Per PIC, pest control company provided the traps but they did not place them in kitchen. Continue monitoring and contact your licensed pest control company immediately when vermin activity is found. Facility is re-opened by this office today. OK to operate. Continue to correct violations dated on 06/26/2025 report.

FOLLOW-UP INSPECTION

Follow-up for routine inspection conducted on 11/12/24 where facility was closed due to food being contaminated by rodents. At time of inspection - facility has signed an agreement for regular pest control service and was receiving service from South Bay Pest Control. Per pest control technician - only some droppings was observed by side door where an opening was also observed. Technician also worked to set up traps throughout the facility. - All areas where droppings were previously noted was observed cleaned and sanitized. No other rodent dropping observed in areas or surfaces of food preparation or food storage. Facility is okay to resume food operations. The following major violations was also observed to have been corrected and still in compliance at time of inspection: - Potentially hazardous foods were measured at proper holding temperatures. See measured observations. - Ice machine has been cleaned and sanitized. - Handwash sink was clear of any obstruction and has warm water, soap, and single-use paper towels available. - Did not observed any adulteration of food. The following was observed still out of compliance: - Pipe under the three compartment sink was damaged and waste water drains onto the kitchen floor. Violation was corrected on site by employee who repaired the drain pipe.

Routine inspection conducted in conjunction with CO0155726 Facility is CLOSED due to vermin infestation and shall cease all food operations. Facility will remain closed until there is no longer evidence of a vermin infestation. At time of inspection, fresh and old rodent droppings was observed in multiple location throughout the facility. - Around the water heater. - Under the three compartment sink - Under shelving by the back door - Under shelving by dry storage - On top of canned food on dry storage shelves. - On bulk storage container - Inside bulk container of flour. - On food prep table next to the food prep unit. - On plates stored on food prep table - Along the walls around the entire front counter - On shelves used for equipment by the front counter - In back room dry storage area on shelves. - On the floor all along the walk-in cooler. It is recommended that facility obtain the service of a licensed pest control. If pest control is used, please provide service report from pest control upon follow-up inspection Facility shall - Eliminate all evidence of rodent infestation including removing any rodents and rodent debris. - All affected areas and equipment shall be cleaned and sanitized. - Seal or repair areas of harborage (gaps, holes, or crevices) Please contact the main office at 408-918-3400 or district specialist to schedule a follow-up inspection to re-open facility.
