Several ceiling tiles are cracked or broken. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

Hose is connected to mop sink with no back flow device and ran to 2-compartment sink. [CA] A hose used for conveying potable water shall be used for no other purposes, shall be clearly labeled as to its use, and shall be constructed of nontoxic materials. The potable water supply shall be protected with a backflow or back siphonage protection device.

Cream cheese was measured at 46F inside prep insert. [CA] PHFs shall be held at 41°F or below. [COS] Cream cheese was moved to a cooler fridge.
