This is a follow up inspection to verify that all major violations mentioned on 10/13/25 inspection report have been addressed. Major violations have been addressed and found within compliance: - K06: Adequate hand wash facilities supplied, accessible. All hand wash stations are fully stocked and operational at the time of inspection. Keep only hand wash station accessible at all times. - K23: No rodents, insects, birds, or animals. No evidence of vermin activity at the time of inspection. Continue to monitor for vermin activity and contact pest control if anymore issues arise. Recommend having pest control go to facility two times within the month for the next 3-months. Then revert back to once a month if no evidence of vermin is seen. Most current pest control reports were provided to specialist for review. - Floor, walls, ceilings: built, maintained, clean. All gaps have been sealed properly around the FRP. Food debris around the kitchen has been clean/sanitized properly. Large hole next to the grease trap has been sealed properly. Continue on working on all other violations mentioned on 10/13/25 inspection report.

The facility is herby closed by this department with regards to having live vermin within the facility at the time of inspection. - Subsequent follow-up inspections after the first follow-up shall be billed $298-hour, minimum one hour, during normal business hours: Monday-Friday 7:30AM to 5:00PM or $645/hour, minimum one hour, during non-business hours, and upon inspector availability. - Notify this department once issues have been addressed. Observed Vermin: Documented in the following areas: - Multiple live cockroaches from different life cycles noted around the 3-Compartment sink - Multiple noted within the sticky traps around the kitchen area. - One food within the freezer chest of the facility. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Samantha K. 4. Notification: The person in charge during inspection, Bunlong Hok , has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

    1. Large gaps noted behind the FRP around the 3-Compartment sink. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Seal FRP back onto the wall properly. 2. Lots of food debris noted around the floor and under equipment at the time of inspection. [CA] Regularly clean under cooking equipment to prevent accumulation of debris. 3. Damaged concrete noted around the grease trap. [CA] Seal large hole around the grease trap properly.
