No soap provided at the only hand wash sink in the back preparation area. Hand wash sink was also blocked by bucket. [CA] Ensure that soap is provided at hand wash sinks at all times during operation. Ensure hand wash is kept free of equipment and available for use at all times. [COS] PIC provided soap.

Food items in cold holding grab and go measured in the temperature danger zone for more than 4 hours. Per PIC, temperatures are taken in the morning and recorded however, temperature was not recorded this morning. [CA] Maintain PHF at 41F or below. [COS] All items removed from unit and placed for pick up from the company that handles the unit. THe unit was unplugged and PIC will provide record of items listed in VC&D report that were picked up by company for credit back.
