Facility will temporarily use one compartment of the prep sink as hand wash sink and use second compartment of the sink for food prep until 12/19/26. Manager has put sign of hand wash sink and prep sink on each compartment of prep sink for temporary use. Manager will email a video/photo proof of correction to district specialist. ROUTINE INSPECTION

Facility was closed on 1/21/25 due to failure to maintain a valid health permit by previous owner. The facility is reopened and a change of ownership has occurred. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: DONBLANC SAN JOSE NEW OWNER: KHAN BBQ INC JAE HYUN CHO The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP14. An invoice for the permit fee in the amount of $1,723.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 5/1/2025 - 4/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 4/4/2025 *Permit condition: THE PRE-WASH SINK AT THE DISH WASHING AREA IS CONDITIONALLY APPROVED FOR HAND WASHING. Note: Facility will be notified after district specialist consults with supervisor if additional hand wash sink is required near warewash area. *Obtain food safety manager certificate within 60 days. ROUTINE INSPECTION

This facility went through change of ownership on November 1. 2024 New Owner: Junghoon Cho New facility name: Oppaya 789 A site visit was made on 12/13/24 to speak with owner. The owner was emailed change of ownership information on 12/13/24 at email oppaya789@gmail.com. Owner was emailed again on 12/20/24 to check the status. Another attempt was made to contact owner via phone call and text message on 1/9/25, however, the owner was not reachable. A permit investigation inspection was made on 1/10/25 to have the owner compelete the change of ownership application. The new owner submitted application and paid the fees on 1/10/2025. An inspection for change of ownership was scheduled with owner for 1/21/2025 at 10:30 AM. Owner stated via text message on 1/21 before the inspection that a new owner is coming next week and he is not the owner. Let the owner know that since he has submitted change of ownership/ facility evaluation application, the inspection will need to be conducted to complete change of ownership process and issue a health permit since he is still operating the facility. Arrived on site, the owner mentioned today is his last day operating the facility, he will close starting 1/22/25. He mentioned new owners are coming to look at the facility and is not aware if they have already purchased or submitted change of ownership application yet. After discussing with supervisor Suzanne, let the owner know that he will have to close the facility today and no longer operate since he does not have a valid permit. Or, he can have the inspector complete routine and structural inspections today and pay the full health permit fees + 25% penalty fees and once the new owners take over, 75% health permit fees will be refunded to him. The owner also spoke to Supervisor Suzanne Lew on the phone and did not agree to closing the facility or going through change of ownership inspection process because he mentioned new owners are taking over the facility. CEASE AND DESIST ALL ACTIVITIES Facility is hereby closed. A food facility shall not be open for business without a valid health permit. The operator closed the facility, turned off the open signed. However, observed him handing out a go to order to a customer outside the facility. Later, the doors were opened, report was handed to the owner. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. Red Placard posted. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied.
