Missing paper towels in the dispensers at the front handwash sink and in the women's restroom. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Paper towels were restocked.
- K23: No rodents, insects, birds, or animals
Fruit flies in bar area, food prep areas and warewash area. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.
- K27: Food separated and protected
Several food items in walk-in left uncovered. [CA] All food shall be covered and protected from possible contamination.
- K40: Wiping cloths: properly used, stored
Wiping cloths stored on prep surfaces throughout facility. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium).

No test strips were available at time of inspection. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

1. Excessive food debris and splatter on floors and walls of food facility. [CA] Walls and/or floors in food preparation area shall be kept clean. 2. Floor in walk-in is severely damages and peeling up. [CA] The floor surfaces shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable.
- K22: Sewage and wastewater properly disposed
Mop bucket had sitting waste water. [CA] Mop water shall be disposed of in the janitorial sink or toilet facility.
- K33: Nonfood contact surfaces clean
1. Food debris build-up on refrigerator handles and sides of equipment. 2. Excessive dust build-up on walk-in cooler fan guard. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Household fridge/freezer combo in use behind the bar. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation).

Ground beef in the prep insert measured at 47F. Per PIC, meat was placed there within the last hour. [CA] PHFs shall be held at 41°F or below. [COS] Beef was moved to the walk-in to cool.
- K39: Thermometers provided, accurate
No thermometers were available at time of inspection. [CA] A thermometer +/- 2°F shall be provided and correctly located in each hot and cold holding unit containing potentially hazardous foods.

1. Several prep units and refrigerators broken and not in use. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used. 2. Front door to facility propped open. [CA] Keep doors closed to prevent the harborage of vermin.
