In the dry storage room, observed dead moths in the rice container. [CA] Ensure facility is kept free of insects to prevent possible contamination of food. [SA] Operator removed dead moths from the rice container.

In the dry storage room, observed an open bag of popcorn and boxes of food stored on the floor. [CA] Ensure all food is stored at least 6" off the floor on approved shelving. All open bags of dry food shall be transferred to food grade containers or placed into a food grade container and covered with a fitted lid.

Observed food safety manager's certificate expired. [CA] Ensure a valid food safety manager's certificate is obtained and available for review.

Observed various open bags of spices and sugar in the dry storage room and on shelving by the walk-in freezer. [CA] Ensure open bags of food are transferred to food grade containers or stored into food grade containers and covered with a fitted lid.
