Paper towels were not maintained inside the paper towel dispenser at the handwash station at the seafood area. [Corrective Action] Ensure paper towels are maintained in the paper towel dispenser.

A follow-up inspection was conducted to verify violations from limited inspection conducted on 10/01/2025 are in compliance. K07: Potentially Hazardous Foods in refrigeration units on Aisle #1, Aisle #14, system C6 and seafood case were all measured at or below 41 degrees Fahrenheit. K35: Refrigeration units have been repaired and are all working properly to maintain Potentially Hazardous Foods at or below 41 degrees Fahrenheit. *Note* Facility has not changed store front signage to Hashi Market. Per PIC, owner will change the store front signage when the interior renovations have been completed. *Note* MY Bakery is no longer operating at this location. Per PIC, a new Hashi Bakery will take its place. Discussed with PIC to submit a change of ownership application to this department.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Hashi Market - Grocery NEW OWNER: Hashi 25 SF LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP08. An invoice for the permit fee in the amount of $1,167.00 (plus possible 25% penalty fee) will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/30/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
