Work with a licensed pest control provider to implement corrective actions. Provide a pest report prior to reopening the salad bar.

    Observed what appeared to be many fruit flies in front buffet area. Observed what appeared to be a couple of fruit flies in the back preparation area. Reviewed pest control reports and services are not provided for flies. Per PIC, they are aware of the situation. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies. Because flies are resulting in the adulteration of food, food at buffet must be covered. If food at buffet cannot be effectively covered to eliminate the potential for contamination, the buffet will need to close until the flies are eliminated.

    MAJOR: Staff found a knife at 3 compartment sink (3 comp used for produce washing per Manager), proceeded to ware-washed/ rinsed it with water, and placed it on prep table for storage without properly sanitizing. Specialist instructed staff to properly sanitize knife. Staff takes knife to sanitizer dispenser and let the dispenser run on the knife for less than 1 minute. [CA]: After ware-washing, manual sanitization shall be accomplished in the final sanitizing rinse by contacting a contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. [COS]: Manager instructed staff to submerge knife in Quat ammonium sanitizer for at least 1 minute. MINOR: Washed and wet utensils and equipment stacked on top of each other at ware-wash area. [CA]: Equipment and utensils must be properly air dried after sanitizing and prior to stacking.
