    1. Raw shell eggs at the cookline were observed left out and was measured at 70 degrees Fahrenheit. Per PIC, eggs were taken out from refrigeration since 11:00 AM. [Corrective Action] Maintain Potentially Hazardous Foods at or below 41 degrees Fahrenheit. [Suitable Alternative] PIC will cook the eggs immediately. 2. Spicy tuna, crab salad, tobiko, green mussel, cooked fish, sushi rolls were observed left out at room temperature at the sushi bar. Per PIC, those food items were made at 1:00 PM and may be returned to refrigeration within 4 hours if not busy. [Corrective Action] Maintain Potentially Hazardous Foods at or below 41 degrees Fahrenheit. [Suitable Alternative] PHFs that have been taken out from refrigeration or have been cooked and will be left out at room temperature will be discarded after 4 hours. 3. MINOR. Potentially Hazardous Foods in the 2 door undercounter cooler and the sashimi display case at the sushi bar were measured at 43 degrees Fahrenheit. [Corrective Action] Maintain Potentially Hazardous Foods at or below 41 degrees Fahrenheit.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Okawa Sushi NEW OWNER: B & W Island Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP13. An invoice for the permit fee in the amount of $1,148.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
