  • K23: No rodents, insects, birds, or animals

    Numerous live flies observed throughout the kitchen. [CA] Use any and all approved methods to abate live flies. Dead cockroaches observed on floor in the hallway outside of the main kitchen area. [CA] Clean and sanitize area of dead cockroaches.

  • K32: Food properly labeled and honestly presented

    Observed numerous to-go containers of ready-to-eat food items without proper labels inside the customer self-service merchandiser refrigerator. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

  • K45: Floor, walls, ceilings: built,maintained, clean

    Observed walls throughout the facility heavily deteriorated and/or stained with grease and food residues. Observed heavy build-up of grime and debris on floors throughout the facility, especially in-between shelves, refrigerators, equipment, etc. [CA] Walls and/or floors in food preparation area shall be kept clean. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface. [CA] Regularly clean under cooking equipment to prevent accumulation of debris.

  • K30: Food storage: food storage containers identified

    Observed in the storage room behind the three-compartment sink numerous barrels of bulk ingredients without labels. In the same room, a bucket of detergent was observed which has the same consistency and appearance of the food ingredients. [CA] Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food if not easily recognizable. Numerous opened bulk ingredient bags observed on the far left side of the back of the facility. [CA] Store open bulk foods in approved NSF containers with tight fitting lids.

  • K35: Equipment, utensils: Approved, in good repair, adequate capacity

    Observed cardboard used to line floors throughout the facility. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent. *** Repeat violation.

  • K22: Sewage and wastewater properly disposed

    Observed wash drain pipe below the hand wash sink completely disconnected from the wall. Waste water observed draining directly onto the floor and through the floor drains. Employee connected the drain pipe into the wall, however, a steady leak was still observed. [CA] Liquid waste shall be disposed of through the approved plumbing system and shall discharge into the public sewerage or into an approved private sewage disposal system. *** Repeat violation

    Hot water at the three-compartment sink measured at 118F. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks. *** Repeat violation

    Hand wash station in the kitchen by the side door of the facility lacked hand soap and paper towels. Paper towels available, but not easily accessible at the hand wash station by the janitorial sink. [CA] Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. Hand soap and paper towels shall be readily available and easily accessible at all times. [COS] Hand soap and paper towels were provided at the hand wash station.

  • K24: Person in charge present and performing duties

    Manager and/or "person in charge" not present at time of inspection. [CA] A “person in charge” who is responsible for operation of facility shall be present at the food facility during all hours of operation.

  • K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing

    Observed heavy clutter of items throughout the facility. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used. All doors of the facility observed opened at time of inspection. [CA] Keep back doors closed at all times to prevent the entrance and harborage of vermin. *** Repeat violation
