ROUTINE INSPECTION
Notes: - An exit interview was conducted with the operator to address the inspection findings. The OIR was issued to e-mail address that was verified onsite. - The facility's kitchen is non-operational, however, EH staff observed a poster stating free tacos are provided with a drink order. PIC stated the tacos are provided by El Guapos (FA0200307), distributed by San Jose Bar & Grill staff, and maintained hot using sternos. No food was observed onsite. Further evaluation may be required.

Repeat Violation: The paper towel dispenser in the bar was empty. Note: The permanently installed paper towel dispenser is on the other side of the bar. Provide a paper towel dispenser closer to the handwash sink and soap dispenser. [CA] Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. [COS] Staff stocked the dispenser at time of the inspection.

1. The coving near the deep fryer is in disrepair. 2. The flooring of the walk in cooler in the basement has an accumulation of food residue. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.

Facility lacked testing materials for their choice of sanitizer, quaternary ammonia. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

Repeat Violation: Facility lacks a Food Safety Certified individual. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times.
