FOLLOW-UP INSPECTION

On site for follow up inspection. Facility was closed due to rodent activity. No observations of any vermin activity in the facility. Facility was cleaned and pressure washed. No droppings observed. Areas with rodent droppings noted during 8/28/25 inspection report were all cleaned. Pest control report from 8/29/25 provided and no activity was observed by technician. Pest control report indicated various holes were identified by technician in the facility. Observed all holes were sealed in the following locations: - under soda machine near fruit prep unit - above ice machine - in water heater closet - under dish machine Per manager, another pest control service is scheduled for sometime between Sat-Mon. Email pest control report to district specialist no later than 9/1/25. Facility is ok to reopen.

ROUTINE INSPECTION

Facility shall cease and deist all food operations. Facility is closed due to rodent activity. Food facility shall remain closed and complete the following requirements before reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider Ensure the facility is cleaned and sanitized and there is no evidence of rodent activity in the facility. Contact district specialist Sukhreet Kaur at (408) 918-3460 or sukhreet.kaur@deh.sccgov.org to schedule re inspection once the above requirements are completed. - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Observed multiple rodent traps under cooking equipment, inside soda machine cabinets (with what appeared to chewed foam/plastic material), in dry storage room and other equipment throughout the kitchen. [COS] PIC moved all the PHFs to walk in cooler.
