1. Observed Vermin: Documented in the following areas: Live cockroaches on 2 sticky traps in dry storage room . Also observed fresh mouse droppings in the same room. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Elizabeth Tobin. 4. Notification: The person in charge during inspection, Monica, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

[COS] PIC voluntarily discarded the compromised PHFs.

OWNERSHIP CHANGE INFORMATION NEW OWNER: DONUTS & THINGS INC FACILITY NAME: DONUTS & THINGS The applicant has completed the change of ownership application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 02/01/2025 - 01/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. Post permit in public view. Structural Review inspection conducted on 01/15/2025 Permit condition: All foods must be prewashed or not require washing. Prior to the installation of any new equipment, submittal is required to Plan Check. Plan check consultation hours are Monday thru Friday 730AM-10AM and afternoon by appointment only at address above.     Observed potentially hazardous food that measured in the temperature danger zone (raw shell eggs 65F and opened jug of milk 47F). Per PIC, the eggs have been out of refrigeration for approximately 2 hours. PIC also stated milk is taken in and out of refrigerator by customers. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Minimize the amount of time PHFs are out of temperature control. Facility may consider using time control (TPHC) as a suitable alternative provided they fill out the Department's TPHC form, time mark the food for 4 hours and discard the food at the end of the 4 hour mark. [COS] PIC put the eggs back into refrigeration to quickly cool down to 41F.

  • K38: Adequate ventilation/lighting; designated areas, use

    Cooking equipment did not have a 6" overhang of ventilation hood. One piece of equipment is about 2 years old per PIC. [CA] There shall be a minimum of 6 inches overhang of ventilation hood over cooking equipment. Move equipment closer together to provide the 6" overhang; remove cooking equipment if necessary.
