OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: MASTER BUTCHER NEW OWNER: MASTER BUTCHERS OF USA INC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $1,580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 11/01/25 - 10/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 10/06/2025 *Permit condition: n/a *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date. ***Prior to the installation of any new equipment, submittal is required to Plan Check. Observed pre-packaged frozen meats in the 3-door grab-n-go freezer lacked sufficient labeling. [CA] Food prepackaged in a food facility shall bear a label that includes: 1. The common name of the food. 2. If made from two or more ingredients, a list of ingredients in order of weight. 3. Quantity of the contents. 4. Name and place of business of the manufacturer, packer, or distributor.

Observed plastic disposable cup used as scoop inside the rice cooker. [CA] Use approved scoops with handles to prevent cross-contamination.

At the kitchen handsink: observed the cold water faucet did not turn on water. Note: warm water measured 105 - 107F when the warm water faucet was turned on. [CA] Repair cold water faucet and ensure sink can provide cold water. Note: PIC stated a plumber was already scheduled to come today.

Observed facility added a new large kebab grill under the type-1 hood without approval from this Agency. [CA] Before purchasing or installing new equipment, facility must first consult Plan Check and obtain written approval. SUBMIT PLANS TO PLAN CHECK BY 11/17/2025. DO NOT USE THE KEBAB GRILL UNTIL IT HAS OBTAINED WRITTEN APPROVAL. See notes below.
