    In the dry storage/equipment storage room, at the installed wooden shelving, observed multiple live cockroaches. [CA] Ensure facility is kept free of cockroach activity. Seal all cracks and crevices to prevent harborage. Contact pest control to provide service for facility. Remove unapproved wooden shelving from the facility. [SA] Operator eliminated live cockroaches. Operator to removed wooden shelving.

  • K09: Proper cooling methods

    In the kitchen area, on the counter-top, measured a container of cooked rice cooling between 110F and 130F. Per operator, cooked rice cooling from an hour prior. [CA] Ensure potentially hazardous foods are RAPIDLY cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in an ice bath, using shallow containers, separated into smaller portions, by adding ice, using ice paddles, and/or using rapid cooling technology. [SA] Operator moved cooked rice to shallow containers and moved into the walk-in refrigerator to facilitate cooling.

    In the kitchen area, at the insert top of the prep unit, measured raw shelled eggs at 49F. [CA] Ensure potentially hazardous foods are held cold at 41F and below to prevent bacterial growth. [SA] Operator moved raw shelled eggs from the insert top to the undercounter compartment to facilitate proper holding temperatures.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: CASA LUPE REAL NEW OWNER: CASA LUPE REAL LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $1,580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit is a conditional permit will be effective: 07/01/2025 - 09/30/2025. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 05/28/2025 *Permit condition: Facility is currently conditionally permitted to operate pending the complete installation of a prep sink through plan check.
