    Observed personal belongings on shelves and hanging on the walls. [CA] Provide lockers for employees to place the personal belongings in. The room next to the office has a mess in it with old equipment and pieces of drwall that is deteriorating with pieces on the floor. [CA] Clean and organize the room.

    The food prep table that the slicer is on has deep cuts and staining beginning in them. There is a piece of bare wood that was added to the prep table that is also beginning to become soiled. [CA] Replace the table with approved type prep table. The soda refrigerator on the counter is leaking. [CA] Repair the unit.

    The floors are accumulating mildew and grime in between the tiles. [CA] Replace the grout where missing/deteriorated in several areas.

    All employees that prepare do not have a food handler card. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire. Please keep them available upon request. [COS] The certificates were texted to the manager.

    Observed wiping cloth bucket at approximately 400ppm chlorine. [CA] The chlorine level must be at 100ppm chlorine.

  • K30: Food storage: food storage containers identified

    Observed the drink holders stored in the unused restroom. [CA] Discontinue storing the drink holders in the restroom.

  • K29: Toxic substances properly identified, stored, used

    Observed the sanitizer being stored in a bottle labeled degreaser. [CA] Do not reuse this bottle for other items. Spray bottles must have what it is labeled as in the bottle.

  • K07: Proper hot and cold holding temperatures

    Observed the soups put into cups since there were small amounts left and placed in the steam table container. The chicken noodle soup was at 126 degrees F. It must be kept in the containers directly and it must be kept at 135 degrees F and above. [COS] The manager poured it back into the container at proper temperatures.

    The hand washing sink has a small bare wood homemade shelf to hold a soap dispenser. [CA] Remove the bare wood shelf and refill the wall mounted soap dispenser.

    The bathroom door is not self closing properly. [CA] Provide the self closing device on the restroom door.

    Not all employees have a food handler card. [CA] Please keep the food handler cards available upon request.

    Observed the light inside the walk in to be very dim. [CA] Please provide a brighter light bulb in the walk in.
