An invoice for the permit fee in the amount of $1580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. 25% penalty charge: $395.00 The Environmental Health Permit will be effective: 7/1/25 - 6/30/26. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. [CA] Install a commercially approved mop sink faucet. 3. Found unapproved hand wash sink installed at cooks line. [CA] When in disrepair, facility shall install a commercially approved hand sink.

1. Measured container of white rice holding at 52F in walk-in-cooler for more than 4 hours. 2. Measured tray of pork/shrimp spring roll holding at 62F in 3 door cold top for less than 4 hours. 3. Measured brown rice holding at 106F in rice container for less than 4 hours. [CA] Maintain all cold PHFs held at or below 41F and hot held PHFs at or above 135. [COS] Operator voluntarily discarded PHFs in trash. [SA] Operator opted to use time as a public health control for spring roll and brown rice.
